i recently, less then 10 hours ago from posting this, engaged with an online Mistress in moderate cbt that was mostly hitting of the balls. She also wanted me to use toothpaste as a "lubricant" for added sensation during sounding.
i have done sounding and cbt before so it was nothing new. But she had me hitting my balls over and over to the point with every hit i started shaking in pain and with the addition of the toothpaste i noticed a bit of swelling around the tip of my penis, and also a bit of blood mixed in with pre-cum. There didn't appear to have any blood in my urine.
Is this bit of blood something i should worry about, and is it safe to use toothpaste in such a way?

A little bit of blood probably means you have scratched or torn the lining, which is now susceptible to infection. Just pay attention to see if it becomes irritated (infected) over the next few days.

If you had been so rough while using cock rings that get pressure up beyond normal limits, you might have burst a small capillary vein (it happens). Same aftercare as above.

(meanwhile at the alternative health clinic)
A man had spots on his dick and his family doctor told him it had to be amputated. Seeking an alternative to getting his cock chopped off, he went to a natural wellness clinic hoping for the best.

The doctor examines him and says, "You've been fooling around with a dominatrix haven't you?"
The man replies, "yes."
Doctors says, "This is a very common problem. Did you see any other doctors?"
The man replies, "Yes, my family doctor."
The doctor says, "I bet he told you it had to be cut off."
The man answers, "Yes!"
The doctor smiles, nods, "Don't worry, we don't have to cut it off.... it will fall off by itself."

Swallowing toothpaste is considered detrimental. Also, I suggest you read the ingredients on your toothpaste and now stop to think that you've introduced that to your urethral tract, bladder, kidneys etc. (Let's hope it wasn't the kind that has the scrubbing micro-beads)

I would suggest, as RS said, that you keep a close eye out for any changes and not be foolish enough to do it again.

Toothpaste isn’t sterile and you don’t shove anything up your urethra that isn’t sterile, including lubricant. Next time a Domme tells you to contaminate your urethra, question her knowledge; it may be that she knows she’s putting you in deliberate danger because she’s a man hater but more likely she’s just an uneducated moron.

There is a big difference between kinky writing and porn and what we do in real life.
Kind of like Maury - someone will haul back and bitch slap the other, and knock them to the ground, and not even leave a red mark.

If you tried that in real life you would at the very least have a big red handprint on your cheek, and maybe whiplash, a concussion, loose teeth - etc etc etc.

You really don't want to put unsterile, corrosive and/or abrasive things into your urethra. These things could cause urethral strictures, and leave you visiting the urologist regularly to have your urethra dilated. Not a pleasant experience!

For that matter, the only thing you really need to put into your urethra is a stainless steel sound that can be boiled vigorously before and after use. And I certainly wouldn't recommend putting it in any further than the length of your penis when it's flaccid. You really don't want to get to the bladder. And while we're at it use sterile lube and clean off the tip of your penis with betadine solution before play. It isn't sexy, but it is safe. And make sure whoever is sounding you washes their hands well before touching anything.

This doesn't sound safe or honestly sane in any regard. Of all my time in the BDSM community (been awhile), I've NEVER heard of someone using toothpaste in that regard.
