You can use a stock avatar or your main profile pic. Those are your only two options.

If you have a profile pic on the collarme side, then you can use that here. Go to the top of your screen, click MY PROFILE, and you will see a checkbox to use your main profile pic.

A 'stock avatar' is a pic supplied by CM. There is a pretty good variety to choose from if you don't want to use your profile picture. I've never actually counted but I'd bet that more people use stock avatars than their actual photos.
Mine is a stock pic.

Go to MY PROFILE at the top of the screen, click on that, and about halfway down the new page is a line that says CLICK TO CHOOSE AVATAR. Click that and you will see pages and pages of stock avatars.

I believe I am having a blonde moment... so please correct me if am. I don't have a check box to say use main profile pic, it offers the option of using avatar as profile pic not profile pic as avatar.

Oh my options, I have "Dont display avatar" as the selected choice, and that allows my profile picture to show. Hope that helps.
