If it makes you feel any better (I doubt it), the same thing happens to men who masturbate too often with their hand.  They get used to the same sensation of using their hand which is quite different than the sensation of being inside a woman's vagina or the feel from receiving oral sex.  It can take them quite a while before they reach the point where they're able to ejaculate again via intercourse alone.
 
The positions and amount of effort required by different muscles are also quite different.  This can make the whole sex thing for both partners (women who use vibrators and men who use their hands) very different than what either is accustomed to.  There are a whole bunch of factors involved based on my experience, including the shared intimacy and dependency upon communication with another for your pleasure that is required for that orgasm to happen.  Hope this helps cast things in a different light and put some additional perspective on the issue from the other side of the coin.

I'm glad I could shed more light on the issue for both sides and how it can affect all.  Sharing any insight I may have is something my Mistress encourages me to do.  Since my relationship with her is relatively new, it's something she and I have also had to deal with and work to overcome. 

I'll also add, as information for Dominants into orgasm denial/control, it makes it even more difficult for many subs to relax and allow themselves to orgasm on their own (or whenever they're ready) as a result of the conditioning they've experienced with present or previous Dominants; something I can attest to as well.  This kind of conditioning teaches them to shut down or numb-out their response to sexual sensations until they're told otherwise; at which time they're expected to turn it back on as if it were as easy as flipping the switch to "ON" for say the lights in a room.   

I feel very blessed to now have a Mistress who is working with me to undo the conditioning I previously experienced; teaching me to it's okay to not have to wait for her permission to ejaculate, which for me, feels a bit as though it's akin to retraining one of Pavlov's dogs...
