Ive witnessed alot of public scenes that get that "oh my god are they really doing that?!" reaction from the crowd. I know quite a few people who play JUST for that reaction. Ive only twice had to physically remove myself from a room because a scene was too much for me...and once it involved the smell of burning flesh and I was pregnant and I dont really think it was the branding itself that made me leave... Only once have I witnessed a DM actually stop a scene because someone spoke out agasint what the Top was doing (involved a medical thing, some doctor stepped forward, Top admitted he'd never researched it, saw someone mention it once and wanted to do it....anyway) I have seen scenes stop for other danger reasons...fire too close to cielings, faulty equipment, etc. If you see something that looks like its a danger, say something to whomever is in charge, they can decide if it warrents stopping the scene, or just stepping in quietly behind the scene and moving something out of the way. Someone up there mentioned they had to walk away when someone was screaming for help. I agree its hard to not want to "rescue" someone begging for a beating to stop, but the majority of the time, even in scenes of that nature, the Dom is "checking in" with the sub in some other way.
