Work place: A person called to collect the sharps' containers. I notified the RN on duty by saying "The lady from ...... is here to collect the sharps", to which a little voice called out, "I'm not a lady, I'm a man, thank you for upsetting my day". 

Another work place faux pas: For a year I had been asking for inter-departmental communication to be raised. On this occasion a family member of a service user called in and I greeted them as usual, then followed up by saying, "Your father will be so pleased to see you today." The lady replied, "Only a medium or sudden death for me will ensure that. He died this morning."

OMG.......my job for a long time was to triage patient's symptoms via the telephone, and give a recommendation. One person called, very pleasant soprano type voice, and I am going through the list of medical questions, and ask, "when was your last period?"; and the reply was, "Never. I am a man." The name was one of those like Ashley, or Leslie, sort of a unisex name. I was really flustered, but apologized, and tried to be cool; right up until the end, when I said "Thank you for calling, Ma'am." **sighs**
