I think going through some very strenuous preparatory procedures, such as have been suggested here, is a very good idea. This lifestyle is littered with subs left behind when 'lifetime' commitments broke up specifically because they did what their D wanted, and then -well, then he didn't like the result.
I'm an alcoholic. My AAOnline homegroup is a group just for people in this lifestyle. I am reminded of a slave on there who had a slip when her Master decided that she would be able to drink normally if he was controlling her drinking. So she would only drink when they were out to dinner together, and he would order her a nice glass of wine. Guess how long that lasted? Soon she was lying, stealing, and hiding her bottles in the bathroom. (because that is what active alkies do. Just like taking a dump is what someone with diarrhea does.)
Of course he had to release her. She was lying, stealing, hiding things and disobeying him...
Think through the consequences of your decisions before you act on them. I am not saying that the two of you will not chose this course of action, but, I know how long a year seems, but it is not that long. It is maybe a good time to start the explorations - see the psche, try the meds, do lots of research, ect.

oooooooooooooooo.  Ok, that Master, is an idiot and obviously did not completely understand addiction.  Yeah she lied and stuff, but the way I am looking at it, is that he is responsible for leading her back to the bottle.  The same reason why you don't take someone with an gambling addiction to a casino, good grief!

this is true.  many t's have done this.  though 99% of them die before they reach the hospital in the ambulance.  due to loss of blood.  these are t's that have serious thoughts of suicide.  its very common in my area.

I wouldn't worry about liking him after surgery---I would want HIM to know the long term health risks and changes to his body!

You might want to start with a complete mental evaluation of you both and then minimum of one year of therapy to determine whether this is something he really wants long term.  Chances are, your relationship won't last that long anyway, but it is less than the minimum recommended standard for someone going through sex reassignment surgery.

He'll still be straight, afterwards, you're not amputating his brain.

Other than that, you'll find plenty of surgeons in various Asian countries that will do this for a good amount of money. If he is hell bent to have this happen, put him on a plane to one of these countries, and have him work it through the local community, there are plenty of extreme Western lads out there. You should NOT be there, because if he has second thoughts in a few years' time, you may find yourself on the receiving end of a major lawsuit, or possibly even in jail.

Not always necessary. If the cancer is is only in the prostate gland then it is partial or completely removed. If the cancer has spread to the testicles, then chances are the cancer is also elsewhere in the body. (lost 2 uncles and a cousin to prostate cancer)
