Had to be more than just a bit drunk that is a lot of tissue to go thru to severely damage the heart and would require a significant stabbing force to drive down into the chest cavity rather than just being an accidental slip

The worst injury I've caused while drunk was head-butting Alpha in his now-retired eyebrow piercing. Man did that bleed! They would have had to be bouncing really hard and exuberantly for it to slip like that... damn. makes my ribs hurt.
