I think a lot of subs eroticize the idea of dual penetration - being violated in his ass and mouth at the same time. I do enjoy it for the sheer objectification factor - if a man is on all fours and both holes are being used, he's pretty much just an object at that point, unable to to anything but be used. Logistically it can be a little hard to put together - you need a girlfriend who is also into the dual-fuck strap on scene, or another man, which then turns it into a "forced" bi scenario also. More personalities, more potential drama.
But for those that want to create this encounter alone, you can do it using a mounted dildo against the wall or refrigerator door, or a tall mirror (better, so he can see what's going on behind him). If you are really going to be giving him a rough ride, he needs to be in fairly good shape, in my experience, so he doesn't get his head slammed into the door.
