I know that we do not feel the pain we are inflicting, but certainly we can see if we are doing damage - usually, at least. At that point, even if my sub won't call it, I will.
Unfortunately we have those times when we can harm without seeing a sign of it. While I can be quite severe, I have hard limits when it comes to risking physical damage - beyond merely splitting the skin slightly while caning, or the like. If it won't heal in 24 hours or so, I generally won't even consider it, no matter how the little bugger begs for it.
