Believe me, I feel your pain! Like you, I've never been able to find a pair of boots (even the supposedly 'wide' ones) to fit. My calf is 24", and even the wide boots usually don't go over about 20" at the most, and that's usually the larger foot sizes. So far, I've yet to find a solution aside from custom-made, but if you're looking to go that route, I can recommend these two sites:
Motor Cowboy - I bought a pair of the ladies' square toe boots last year, and I love them! They fit beautifully, and while they weren't exactly cheap, they were worth it
Siam Leather Goods - haven't bought from them yet, but they say they can make just about anything you design, so I'm working on something for them
