This is going to sound like a copout but I really don't have a defination for it. I tend to accept how other people define themselves as in or out of it unless I have specific reasons for doing so - such as if my personal, real life interaction with them leads me to believe that they aren't so much kinky as using any means they can to get laid.

Beyond that... it's more of a combination of taking people at their word and "I know it when I see it".

As for when I started to consider myself in the lifestyle... I really can't pinpoint it. I started getting my hands on BDSM-themed porn when I was very young but I didn't know what exactly I was looking for. I just knew what I liked. When I was 18, I met a dominant my own age who started to open the way for me. From there it's been almost a natural progression so I really couldn't say where or when. Maybe when I first scened... maybe the second time... maybe when I first wore a collar... I just don't know.
