The issue is a form of rubber contact dermatitis. It can be caused by a lot of things, from the accelerators that are used to actually synthetically prepare the rubber, to the different chemicals that may be infused with dyes to give it the color and whatnot. Because you've washed it with soap, I'm willing to bet that it's not anything particularly on the rubber ball gag, but more so a chemical compound that was put into the rubber ball gag when it was harvested and manufactured. Your best bet, is to get rid of the ball gag, and either buy another one and HOPE that it doesn't have the same effect, or find an alternate form of gag that isn't made of rubber :)
