It is progressive, and as far as i know, irreversible. It can be surgically fixed - basically, what the surgeon does is an incision much like a circumcision, pulls the skin down and puts in stitches into the tissue under the skin to straighten the penis out. I've been in on a few of these, and it seems to work well. There isn't any reason why surgery should inhibit erections. Actually, diabetes by itself sometimes does a good job of that, so if he smokes or drinks excessively he needs to quit if he wants to keep having intercourse. On the other hand, there are certainly different things to help with erections - up to and including penile prosthesis's. Complications of surgery would be the usual - infection, loss of sensitivity (very rare since the surgeon isn't interfering with nerves), death - also very rare. He would swell up like a poisoned pup and would take about a week for the swelling to go down. Basically, it would be like having a circumcision - which for an adult isn't desperately comfortable, but eminently survivable.
