Male doctors up here routinely use lidocaine (aka xylocaine) in order to last longer. Just remember to rinse it off after a few minutes, and be on the lookout for sensitivity reactions the first time around. Keratinization is a non-problem, as the uptake is still going to be more than sufficient, but it might need to stay on longer, depending on the overall degree of keratinization there. As usual, a creative partner will be able to come up with a million entertaining ways to determine the point when it has started working sufficiently.
