I have been through major nipple issues in my life entirely due to breast feeding. This can happen when your first child needs to nurse every 2 hours for 40 minutes at a stretch ! IMO you most definitely need to let the area breathe. I know, not exactly easy to let the nips hang out, most especially when your breasts are milk filled. Wear your nursing bra and open it up to let the nipples breathe as much as you can. Also, warm compresses should aid healing. A cool compress (or ice, if you can stand it) will relieve pain, and I'd try this before I pumped, since that has to be hell for you. Not sure how your cold nipple will react to pumping, my breasts were so filled with milk, it wasn't an issue when I had to pump. Hope this helps.
