I recently enjoyed delivering a nice caning to a rather maso young lady. As we progressed she kept asking for more. No matter how hard I struck, no matter how freely the tears flowed she demanded more. I reached a point where I knew I had to back off or risk permanently damaging a very sweet pair of buttocks.

I raised large welts but never drew any blood. Can anyone give me guidance on what the limits are here? I've seen videos where blood ran down the lady's legs. I'm sure she ended up with permanent scars.

How can one avoid permanent scars?

I've run into this same problem with intense nipple play. Some subs find the pain so intoxicating they keep pleading for more even though I'm about to do some real damage to a very sensitive part of the body.

Its actually very difficult to permanently scar the buttocks. I have caned people until they are bleeding and two weeks later theres no sign I ever caned them at all.
I would be very careful about a sub demanding you give them more though. Are you dominating her or catering to her demands.
Nipples are different. The skin of the nipple is rich in nerves that are very close to the surface. Torturing nipples on a regular basis can de-sensitise them and even deform them permanently.
I have never actually seen a woman with deformed nipples but I have seen some male subs with really strange deformities caused through years of nipple torture.

I'd just like to add that permanent damage aside, the skin on the nipples is delicate and thin, and once broken, prone to infection. Infections in the nipple can travel into the milk ducts and make a woman really ill. As someone that has experienced it, I strongly advise close attention to hygiene if there is the remotest chance of broken skin.

Caning though, I can't help. I'm a huge wimp and so none of our caning scenes have come remotely close to drawing blood.
