Definitely a model release, which explains it's not for profit, confirming the age of all participants, with copies of their ID's. The contract will explain that the information contained, including the personal information of the participants, is privileged/private and will not be released under any circumstances.
The bold portion, isn't exactly true.
If you are required to show the 2257 paper work for legal reasons, that information is not private or privileged. So putting that in the model release would be silly.
Another question, because I am nosy.
If this isn't for profit is it for promotional use for other ventures of yours?
Sorry, call me jaded but not many people "produce" movies for free.
And even if you are using it for promotion of yourself or another business, IMO it is still a for profit video. *shrugs*
