We're debating Beauty Parlor Syndrome over in another thread, where bending the neck back for too long has been linked to causing strokes from blood-clots in the brain. They're still arguing about the odds of it happening.

Kink.com with their young skinny models do it up to 3-5minutes at a time, though use scene breaks to make it seen longer. Some fiction had it going on overnight.

Would you do it with a sub?

When I was still in high school (don't worry, this isn't going to be an underage story) a teacher had this happen to him whilst supervising a school trip rock-climbing. He was standing at the bottom watching the students climbing and he suddenly collapsed. He hit his head on a rock and was left with serious enough brain damage that they never expected him to recover the ability to walk or talk. It was explained to us as 'Sistine Chapel Syndrome' but sounds like the same thing you are discussing. He was in his forties, fit and healthy. Extremely sad.

Anyway, it seems to me if it can happen to a healthy guy doing a low risk activity, it's one of those struck-by-lightning type of bad luck incidents that could happen to any one of us. I wouldn't be interested in any sort of long term uncomfortable bondage situation like keeping the head forced back overnight, but it would be more due to worries about less serious but more common injuries - strains, pulls, tendon and ligament damage etc - or possible breathing difficulties. There are risks we can and should seek to alleviate, and unfortunately there are those things that we can't anticipate until they happen.

So no, I wouldn't do it for long periods but not necessarily because of this.

My neck muscles cramp up quickly in this kind of position. So I've never had to maintain it for more than a couple of minutes.
My recollection of beauty parlor syndrome is that it takes 20 minutes and won't happen unless you're older and/or have other pre-existing conditions. Unfortunately you don't know that you're at risk of a stroke until it happens usually. You can have plaque in the arteries and be fine until you aren't.
