Nipple to nipple is not 'cross chest', it's 'co-plain' and 'allowed'. Cross chest (Front center to back center or side to side centered) is a frequent caution with TENS range play and EMS (Electrical muscle stim) stuff, however there are ZERO reports to OSHA, FDA (which regulates medical equipment) or other Federal consumer/safety agencies tracking deaths and technology mediated hospital admittances.
If you Want cautions, Wikipedia>TENS has the longest list I've ever seen, and a not very convincing reference for them. The people who make TENS and their so far tolerant legal advisers Don't mention most. It leaves out placing TENS patches over the carotid arteries, a not rare caution, as spasms of the muscular sheath of these neck arteries are thought by medical professionals to possibly cause loss of consciousness by interfering with blood supply to the brain.
TENS get used therapeutically above the waist in MOST instances, literally millions of people and times EVERY DAY. Incredibly more often than for sex play. Strange how such a dangerous practice has zero body count?

One extremely well-known heart surgeon simply gave a blank stare when asked about TENS involved heart incidents. He's never heard of one either. JUST internet forums know about this?
TENS machines usually top out between 70 and 100ma. (.1Amp) If you don't know what an Amp is or what it means, your are trusting whatever 'expert' you ask? Electro stim equipment sold for sex play is usually less powerful but may use different profiles of frequency/voltage/amps than familiar TENS units. Stuff that doesn't have a UL rating and full disclosure of power and pulse profile might be avoided by the cautious?
Nipple to nipple feels slightly different from across a single nipple. Plain TENS patches are quite boring. Point contact electrodes are far more attention getting? Area of contact matters. Home-builts are used here, wood safety pins with push pins in each jaw sticking out enough to feel but not penetrate skin, electrode wires attached to each push pin metal. With a 2 channel TENS, they can be wired to cross from nipple to nipple (One polarity will feel more intense than the other, + vs -) or across a single nipple for each channel. Which is interesting if each channel is slowly matched to the other in a 'race' to full intensity in the smallest steps possible. Most find full setting too strong initially. Ramped up slowly, full power is easily tolerated, even boring after enough accommodation.
Pulse frequency is a major variable. Single pulses at high power satisfy those on either side of sadism. 300f/sec feels like a vibrator. 8-12f/sec has been popular for sexual stimulation of nipples here.
