This is an example of pretty much everything NOT to do in a scene. Drugs were involved. He was bound for an extended period of time. The other two left him alone, in the bondage, for extended periods of time. They got back and assumed he was asleep. It wasn't until much later that one of them got the idea to call for paramedics, and THEN tried CPR. (It's unclear if he was still bound at this time.)

Now the two are on trial for manslaughter charges, which they deny.

They did commit manslaughter , intentional or not, they did kill him.
This is not the first one, and wont be the last one.
You can make a list of people killed by kinky stuffs. Even a buttplug can kill you.
Some go to extreme, that they actually kill people intentionaly while in a bdsm session because they like to see the victim struggling for life. They are serial killer that use bdsm .

Hemorrhoid laceration followed by internal bleeding until the subject goes into shock unnoticed because the Dom left for a quck trip to Taco Bell. This stuff happens a lot and the fact that the bound subject is left alone is the reason a small problem becames a fatal one.

AS you all know, nobody can leave a bound subject alone. Should you need to leave for a minute another person must stand in or you release the subject. Always keep medical safety sissors within close reach so you can cut the subject loose when needed. Water must be given the subject every 30 minutes and she must lick your fingers. Ice is acceptable, in a large bucket. Ice and water must be close by should you be using fire in any form.

I once dropped a bound person on the dildo because they were too large for me to easily move myself.

I learned a lesson, to leave THEM some maneuvering room.

They learned that I'm not so good at bondage toys.

This says it all. It should be a crime to leave someone alone in bondage as it is the number 1 cause of death/injury from bondage.

I'm sure you could make a case for reckless endangerment, in which case it's already a crime.

most of what I wish to say has already been addressed. This is always a possibility when so called "Dominants" describe themselves incorrectly with a pronoun that simply acts to excite the little head between their legs.

You can't be a Dominate unless you take responsibility for your actions...
