ended in hospital and they kept me, did something weird to my shoulder and popped it out, the x-ray said the shoulder isn't completely in the socket, kind of neither in nor out, CT indicates soft tissue and a calcium build up got in between the ball and the socket of the joint, the specialist (orthopaedic shoulder surgeon) is on vacation this week and nobody else wants to touch it, but so far I heard that they can try to rinse it out or try keyhole surgery.

just want to be a bit informed about the pros and cons of each one, as i do tend to distrust them if they tell you how painful it is, as i learned that for them "some discomfort" means "effing off the scale pain"

Sounds like a long-standing problem if you have calcium buildup in the socket. From a pathology point of view (my specialty) I'm thinking it's the soft tissue such as tendons or synovium that has calcified a bit from chronic irritation.

In any event, I'm very confident that keyhole surgery either to "rinse out" or debride the calcified tissue would be in order. Open surgery is highly unlikely unless the specialist finds that the joint capsule is messed up and he/she needs to repair it. Doesn't sound like that, though.

As a veteran of multiple orthopedic surgeries myself I can confidently say that post-op "discomfort" is total bullshit. It hurts like a motherfucker!! Demand powerful narcotics!! [:D]

Two years ago I had a torn rotator cuff repaired and a subacromial decompression done arthroscopically. They sent me home with a nerve block pump that dosed the joint with bupivicaine for three days to block the nerves in the shoulder. You spend three days with your arm hanging in a sling like it's dead meat but there is zero pain/feeling at all. When the pump had run it's course, there wasn't really any pain at all until I started pushing it in PT and that was negligible.

Good luck with whichever course you choose.

Actually the calcium deposit wasn't in the socket to start with, we have some kind of a lodger, a nice guy who is homeless, but he is a bit of a hoarder, and for some obscure reason he brought old tires into the backyard, I managed to fall over them, wretch the arm out of the socket while trying to break the fall, when it slipped back in (annoying sound it makes, a horrible POP) the calcium deposit seems to have gotten pushed in and trapped between ball and socket, there was a discussion of dislocating it again and trying to push it out, but all the orthopedic guys went "Well, in theory, but we're not going to attempt that..."

Funny thing is I put medical leeches on and thought they are a bit of a failure, as the only managed to kill the pain for about 3 hours, usually the pain relief works for weeks on end, months even... However the morphine they gave me wore off after an hour...

At the moment I'm on 5 different pain killers, though the tramadol they give me and which I should take every 4 hours I prefer to only take when I'm in real agony, as it makes me feel seasick and zombified. I tend to have really odd reactions to painkillers, most have terrible side effects, some simply don't work and they managed to track it down to a genetic problem, some my body refuses to absorb and some have the synapses misfiring, I'm kind of lucky to have a fairly high pain threshold, or I'd be really effed up. Morphium works but has me puking, codeine kinda works, however co codamol sends me loopy, as in panic attacks and manic twitches, MJ is not legal in the UK (not that it would bother me much) but I can't inhale it or I get asthma and again sickness, eating it is difficult to dose and again, nausea... With novocaine just absolutely no reaction... As my doctor joked, I just don't have it in me to be a real addict... The last few times I popped the shoulder out, they put it back in without painkillers, because I said I rather deal with the short pain than the after effects of the pain killers. A few minutes of agony versus days of puking...

It makes surgeries and injuries kinda "interesting" but not in a good way...

I will ask about the pump, seems that might work for me, since most pain killers just really really mess me up, I have the dead meat in a sling at the moment, only with pain, so without pain would be a significant improvement

If you arent a candidate for an indwelling catheter/pump petition the anesthesiologist for an interscalene block with PF decadron--it can help mitigate the pain for upto 24h
