I'm reminded of the old joke, told by Buddy Hackett (Please remember: This was in the 70s so, some of the references will be dated):

A guy spends some time with an oriental lady of the evening. He wakes up the next day and his unit is all different colors.

While it may be a legit thing, the average person does not know fuckall as to how to do it properly. My early comments to the OP was to seek professional help.

My later comments reflect that I believe that there is more wrong to this relationship that transcends BDSM and goes way deeper.

This shit is dangerous, and potentially lethal if not done right. So, this is not a casual, well 'my kink isn't your kink' thing, it is life and death shit that needs to be addressed.

Don't write something off under the guise of 'kink' simply because it is a 'thing'. Removal of a body part done by nothing less than a surgeon is radical... that someone with zero training in medicine wants to do it... well... I'll let you draw your own conclusions from there.
