I generally never ask for anyone's advice other than my dom and good friends.
However, I have a decision to make here real soon.

A little over a year ago I fell off of a ladder.  I still have an enlarged area on my butt from hitting so hard.  Both elbows are still messed up...I'm told people heal at different speeds so if it takes 20 years to use my elbows again so be it.
Also, my pinky on my left hand was dislocated.  They put it back in place at the time.  Yet it has never healed properly.  Which is why I am so typo ridden anymore.  It also affects my ring finger as it is now permanently 2 sizes larger than it was before the incident.
My pinky will not straighten out.  Now my docs in California told me there was nothing that could be done to help me. 
I've recently moved.  These docs want to operate.  I'm told I have some arthritis starting to form in my joint.
This is what I want to happen.  I want the swelling to go down.  I'd like to be able use my hand without severe pain for extended periods of time.  I'd like my finger to be straight again and I'd like for it to stop messing with my ring finger.
There are times I am afraid my wedding ring is going to cut off all circulation.

Now, they can do a mini carpal tunnel surgery.  Though..the swelling could stay forever.
Pain...if arthritis is starting to form it may never go away.
There is an entire joint replacement surgery but it sounds extreme to me for just a finger.
Rehab time? 

What to do?  I have no idea which way I should turn and just looking for what others would do in the situation.  I realize it will come down to my decision though I am so unsure at the moment.

Any thoughts?

Is this surgery being offered by an orthopedic surgeon or a hand surgeon? If it is on the reccomendation of a general orthopedic surgeon, I would sugest asking for the name of a good hand surgeon and consulting with him/her before proceeding any further.

The following is just MYopninion and the way * I * would handle it. I would not let a general orthopedic surgoen operate on my hands. An entire sub specialty of hand surgeons has developed and they are state of the art, most advanced options, and most proficient in this type of surgery. Also they can answer your questions with a greater degree of certainty than a regular orthopedic surgeon.

I think seeing a hand surgeon is the best advice. I hope you have a good outcome.

Internal referred me to the ortho dude.  Ortho dude referred me to the hand dude whom I saw today.  Currently we are on hold for a month.
It appears two of my fingers are broken....the one that does not straighten out anymore.  Well I have this nifty little traction device I had to wear 4 times a day.

If after the month...I guess we re-talk about surgery.
Perhaps if after the month my finger will straighten out that will be enough.
I think I type pretty well for having two broken fingers....and it only took 4 doctor's reading 4 sets of X-Rays for one to read the broken bones.  Wonderful medical system we have in this country.

I think I'd want to leave surgery as a last resort. Have you looked at any other possibilities - some intensive massage therapy on the affected fingers (can sometimes work to break up scarring connective tissue which is causing contraction), or physio of some kind, plus or minus anti-inflammatory injections into the joint? Heck, they shoot up horses' hock joints with gold and it seems to work pretty well (not saying you're a horse there, its just an example!!).
 
Good luck - I bet that was a whole new definition of "ouch" when you took the tumble! Eeeks!

I had my left hand crushed in an work related accident... my hand was pulled into a conveyor system. No bones were broken but a great deal of soft tissue damage was the result. They started rehab immediately... however it was so excruciating I refused to go for a few days after the first two sessions. I ended up going to therapy for over 3 yrs. I had surgery almost one year after the date of the injury to remove scar tissue development & to correct nerve damage. It was like a carpel tunnel surgery.... only that they had to cut way up into the palm of my hand & way down past the wrist as well as open up the top of my hand.

Amazingly enough I basically have no limitation with the hand or usage & the scarring is not so bad. The burn scar of wear the conveyor burn into my arm is far worse than the surgical scars.

However... I dislocated a pinky-finger many years ago doing something stupid... I took a swing at my little brother & when he ducked I clipped the edge of the wall. OUCH! The finger never healed right & there is a slight bend right at the knuckle.

Kind of weird how such a major injury healed so well & silly accident didn't.

Good luck with your decision... my word of advice. If you are able to live with your current status & you are relatively pain free & functional.... might be best to leave it alone because often the outcome of surgery could be worse. This is what all of my doctors told me & why I held off having surgery for a year & flat out refused the skin graph on the area that suffered the friction burn.

I had cut My thumb through the tendon and it nicked the bone.I went to pysical therepy to get My thumb to stretch the tendon...Now the pruple thread has worked its way to a lump under the skin...no real tiredness either so thats a good thin.

I wont lie to you it hurt like a son of a gun.

I didnt have ony one with Me...Maybe next time I wll.

Haven't looked into anything really yet.  I have some sort of nerve damage in my one finger.  Can't really feel a whole lot unless I strain it in some regard.  Then it hurts.  It hurts when I take the traction device off and the circulation rushes back in.
Massage therapy..I can look into that.  We are a year out already because of the incompetant doctors so all in all I'm not sure what they can do.  Though I must give them credit for trying.
Though it is good also to hear Passions story of a successful surgery.  I'm told that if they need to do it...it will be quick very quick.
In the meantime I'm coping as best as I can.

I would have said what thinking kitten did - get to a good remedial massage therapist, they have prevented the need for surgery in so many cases.

Whenever an injury occurs scar tissue forms, its the body's way of healing itself. This scar tissue can't be seen, (its inside)..but will make the injured area painful and harder to move. It has to be broken up.

The swelling (you have to get rid of the swelling, its hard for anything to heal when its immersed in a bucket of fluid) can be reduced by massage too that you can do yourself, gently stroking over the effected area in the direction of your upper arm ( direction is important), every 6 strokes or so, move that arm across the front of your body using the hand of the other arm press your arm towards your body until you can feel a pressure occuring around your armpit area (this area is a drain zone, what your doing is clearing it so that the fluid your moving from your hand can disperse quickly). (hope I described that ok, its basically lympahtic drainage massage).

Hope all goes well.  And before making a choice for or against surgery remember that Orthopedic surgery is the most invasive/brutal kinds of surgery there is and many doctors know this. 

If the problem is skeletal in nature I really don't think massage therapy is going to do much, but with scar tissue and such I would feel work wonders.

Now for the person who complained about our medical system, it could be worse.   Just ask two of my friends who lost relatives to cancer because they had to wait to get diagnosed with cancer, and this was Cananda,

I would recommend seeing a Certified Hand Therapist.  A CHT might be able to assist you with achieving more function before resorting to surgery.
