Anal hooks are a sturdy stainless steel that looks almost like a very large version of a fish hook. However there is no barb, and it is worked to a very smooth finish so that there won't be cuts or abrasions. Normally the long end has a metal loop or ring at the end.
I have most often seen them used where a rope or chain is hung from an attachment point, the short end is inserted to the anus or vagina, and then the ring is attached to chain or rope, lifting the bottom up onto tiptoes to attach it. It gives the fun of insertion and the top can either make the chain simply snug while the sub is on flat foot...then have the sub bend knees slowly to experience higher pressure, or if the sub is on tip toe the sub will feel more pressure when their muscles give out.
They can be incorporated into bondage ties, or the attachment can be movable so that it can be raised or lowered in scene.
