This works out great usually because my oral sex technique usually involves mostly just concentrating on the head during the process to provide lubrication and extra sensation with the hands doing most of the work.  Once orgasm becomes present, I simply remove the mouth and things work along the same.

Of course, if a master or slave find some significance in swallowing cum directly from the cock, it can become a problem to work through.  For me, I generally consider swallowing to be too high of a risk for me (since I'm not monogamous and neither are my partners) and there's never been any "special thing" about it for me.
