Have any of you ladies ever had an ovarian cyst that ruptured? I am dealing with a situation with my sister where that is what happened and what is being done and said as far as her care is hitting me wrong and worrying me.

It's not a matter of encouraging her to get a second opinion. I can simply take her to get that. I have authority to do so. I'm more wondering what was done for others who've had this same thing happen. Hearing other's experiences, since I have no personal experience with it, will help me determine if my reaction is overreaction or not.

You don't give details, but it's not unusual for ovarian cysts to rupture spontaneously and they just heal without medical intervention many times.  It's happened to me a couple times.

Other than that, can't really give advice (and really shouldn't anyway) without more information about the case.

Try searching sites like WebMD, etc.  They're usually helpful to a degree.

That's what happened windchymes. A small cyst on her ovary spontaneously ruptured. She was walking bent over almost in half and saying her tummy hurt a lot, so Mom took her to ER to find out what was wrong. Found it on abdominal CT. They gave her Vicodin and Ibuprofen 800mg and sent her home saying she'd feel better in a few days when the cyst finished draining. They also said there is a very slight touch of peritonitis as well. It just bothered me, still does.

As windchymes said, it isn't unusual to have cysts develop and rupture without any medical intervention at all. Peritonitis is an infection, which I'm surprised they aren't treating with an antibiotic, but their treatment of the ruptured cyst is pretty standard.

It sounds like this just happened, and by tomorrow she should be feeling much better. If she is feeling worse, then get her to another doctor. You didn't mention how old she is, so I don't know whether or not she has been regularly seeing an OB/GYN in the past. Now that this cyst has happened, though, I would say that an appointment with one is in order.

She is in her mid forties and moderate to severely mentally retarded with signs of autism among other things. Her mental level is roughly that of a 4-6 year old. Yes, I agree that maybe she needs to begin seeing an OB/GYN. We've not taken her to one to date because there have never been any indications of problems in that area and she would need to be nearly sedated in order for them to be able to properly examine her. With this happening though, it needs to be done.

It appears to have started yesterday morning and Mom took her early this afternoon after I stated she needed to be seen since she was no better. I thought she should have been seen yesterday, but it was decided to watch and see if she got better.


Edited to add: That's one of the things that is bothering me, that they didn't put her on antibiotics for the peritonitis. I am going to call her reg doc on Monday and discuss it with her and if I don't think she's doing well enough I'll take her back to the ER myself.

Literally translated, "peritonitis" means inflammation of the peritoneum, which is the lining of the abdominal cavity. That's usually caused by infection, but isn't necessarily.  (Like "hepatitis" literally means inflammation of the liver, but it's become the catch-all name of the disease caused by the hepatitis viruses) 

I'm also surprised they're not giving her some antibiotics as a precaution, but if she doesn't have a moderate to high fever, antibiotics probably aren't warranted. They are probably just referring to her peritoneum being inflammed and, therefore, sore enough to make her walk that way, but it's probably not a true infection of the bacterial sort.  Although this could occur at any time.  If she spikes a fever, then that's cause for immediate medical attention. 

I'd also bet that she'll be feeling better in another day or two.  Just for the record, the last one I had, about a year ago, had me lying in bed, sweating and panting like I was giving birth again.  I was up and around that night, and went to work the next day. The first one I had had no symptoms except my periods stopped, and the doc had to give me a big dose of hormones to start them up again.

~laughing~ Don't get me started on the fun of being a girl! I got my own "fun times".

She didn't give any indications at all of any kind of problem until yesterday morning when she came into the kitchen walking bent over almost double and whimpering. As far as I am aware, she's not running a fever, just really hurting. Says the pain meds aren't helping yet. Of course getting her to just lie down and rest is an exercise in itself as she is nervous and constantly getting up and prowling the house, looking through cupboards and looking out doors and windows. If she'd settle down and relax it might help.

Research PCOS (Polycystic ovary syndrome) and see if she fits into that category.

You've already gotten a lot of good input. I'll just add that in my early to mid teens, I had ovarian cysts that ruptured. It was incredibly painful, so much so that I would sometimes just throw up the pain meds after ingesting them. I had several trips to the ER and exams at the doctor's office and the consensus was that the cysts were growing and about every other month would rupture and there wasn't anything to be done about it. I had a doctor put me on special vitamins in an attempt to resolve the problem eventually but what did resolve it in my case, as is sometimes the case evidently, was getting pregnant. The cysts stopped for me after that. In a lot of cases, though, it isn't a pattern as it was with me and just happens and resolves on its own. I know, for me, it really was a childbirth degree of agony. I hope it passes quickly for her! You sound like a very loving sister. Thank heavens she has you.

Because of your sister being developmentally challenged, with the mental level of a 4-6 year old, you have to keep in mind that she will also deal with pain as one that young as well. I don't mean that to sound harsh at all. But think of your little ones and how big a deal they would make over a scraped knee? This pain is definately much worse, but she still will likely have an exaggerated response. As wyndchimes said, if she isn't spiking a fever, she should be ok in a couple of days.

As for not taking her to the OB/GYN....I realize how difficult it must be to have her properly examined given her condition, however, just because she isn't sexually active doesn't mean she shouldn't be having pap smears, mammograms and physicals to maintain her female health. Even though she would need to be sedated, exams catch problems while they are easily treatable. It really is something that will need to be taken care of and I'm sure that her regular doctor will have recommendations about the easiest way to achieve this.

Your sister is definately lucky to have you in her life watching out for her. Not for nothing, but I'm sure she is also enjoying the extra special attention she is getting right now. Perhaps playing a game with her will help settle her down?

Agree with most of the above. Ruptured ovarian cyst hurt like crazy and can reoccur. Usually they resolve and the pain decreases accordingly. I also agree that she needs to be examined regularly, even though she isn't sexually active. She still can develope ovarian or cervical cancer, and the treatment for those is miserable!

As long as she isn't running a temperature just watch her. And do i need to add that you need to actually have to take her temperature, not just feel her forehead? :) I can't tell you the number of people i would see in the ER who would talk about their elevated temperature and never actually have taken it!

Yes, and I've been to the ER for them more than once.  There isn't much to be done for them other than give her painkillers to take when it happens.  Ruptured cysts are incredibly painful, but they aren't life threatening.

I had 3 cysts rupture on me and it was not a picnic.  Having fibro on top of that, you can see that I have a low threshold of pain.  It took having a hysterectomy and the right ovary removed for me to bounce back (it took me 2 weeks to recover from a ovarian rupture at a time).

Actually, it's not like you'd think as to her reactions to pain. She has an extremely high tolerance for pain so if she is whimpering or even mentioning that she hurts to someone, she is hurting very badly. Most pain just doesn't faze her a bit.

Now that she has been on the pain meds since yesterday, she is up and about and doing her thing and only very rarely making indications of discomfort.

You can't keep her in bed in the hospital after surgery either. She can't stand it. She had surgery done on both her neck and her lower back a couple of years ago and she rarely complained of any pain after either and was getting up frequently, or trying to because I stayed with her in her room and managed to get her to settle down and rest more than she would have otherwise lol. It just honestly takes a lot to get her down for any length of time.

Yeouch! I so sympathize. Not good pain at all. I'm glad you were finally able to get it resolved, though what a way to have to go about it!

~laughing~ Um no you don't. I have 4 kids of my own and I only check their foreheads to see if they are warm and then I go for the thermometer to determine exactly what's going on. Feeling forehead will not tell you just how much of a fever they have.

Oh and yes she is definitely enjoying the extra attention. She doesn't get into games though, she has other things she likes so I just go with that and its all good.
