That's really two different questions.

There really isn't anything such as a 'slave license' that means anything. There are some folks out there who get a kick out of that 'slave registry number' thing that you'll see on some people's profiles or in their signature line. Again, doesn't mean anything. There absolutely are such things as dungeons. A dungeon is usually what people call a permanent building, with BDSM furniture, that is open to the public. A lot of major and some semi-major cities have them. There are also conventions that have their own temporary dungeons that are up for the duration of the event, small munch groups that have their own play space, etc. Some individuals, such as myself, have their own that aren't as big as what you'd see at a club, but do a dang fine job.
