It would be a fetish (by dictionary and DSM4/ICD9 definition) if you are aroused by the cigar or the smoking of it without any other sexual stimulation.

I think (though I am certainly no expert) that psychologically a fetish is the sexual excitment or stimulation of a person aroused by an inanimate object that has (biologically) no function in sex. Using this definition, a person who is aroused by shoes would qualify as having a fetish, but someone into spanking wouldn't. Are not spanking, ass play, strap-on penetration, body worship, roleplay, nipple torture, hair pulling and control etc, not merely sexual kinks?

You're correct. Much of what folks are listing here are KINKS and not dictionary-defined fetishes.

Me personally, I have a fetish for cigars (smoking them), uniforms (someone else in them), cops & military men, and exceptional service (of a non-sexual nature.)

As for the question about how does a fetish develop, it is generally brought on at a young age, generally from being told something is WRONG or BAD and then slowly growing to like it. There's always a few instances in your childhood you can remember that hooked you into a fetish. Feet are a such a love/hate object of a womans body. You are always meeting women that hate feet and when you're young you often wonder why, so when you experience them for yourself it somehow turns into a turn on for you that you can't describe to anyone else why it is you like it, but you just DO.
