I really wish I had a lifestyle friendly doctor that I could talk to, I have tried to talk to my doctor, but she wigs out about our family being poly...so I could never bring up BDSM with her.  

I was in an abusive relationship years ago with a very sadistic guy where there was much anal play w/out lube.  It was very painful and I bled a lot, sometimes for days afterwards.  At the time, I just thought that was how anal sex was supposed to be like.  Needless to say, I have a big phobia about any anal stuff now.

That part of me never felt like it fully healed and I have had intermittent pain and bleeding ever since.  My doctor told me that I now have a large hemorrhoid and it will take time to heal.  She said I could go to a protocologist, but they are likely to want to operate and the thought of that terrifies me beyond belief.  

My question:  Is it possible to have scar tissue down there?  Could scar tissue lead to hemorroid or anal fissures?

It sounds like you have a hemorrhoid as that's how they seem to operate.
Hemorrhoids come from a variety of sources, bearing down when your bowels move, having kids, carrying heavy loads, sex, and sitting on cold stone surfaces can draw them out to name a few.
More than likely you had the hemorrhoid before the anal sex and that's why you bled.
The surgery is very simple and not very invasive.
They just clip it off and cauterize the wound to stop the bleeding.
It may not be fun to have a procedure on your butt done, but it's worse to run around with a hemorrhoid making you miserable.
I'd suggest you have the hemorrhoid removed.
You'll be better by the end of the week vs dealing with the thing forever.

Are you able to change doctors in order to find someone that you feel more comfortable discussing your health concerns with openly? As onestandingstill mentioned there could be a number of reasons why you have a haemorrhoid or fissure and most of those are non-kink related.  Anal sex is practised by vanilla folk also and whilst I totally can understand your hesitation to discuss it further or get more information from a specialist it sounds like this problem is worrying you quite a bit - getting a medical opinion may at least give you some further information and options.  Yes they may suggest surgery however you will be given time to make a decision. You have mentioned a phobia about intermittent pain and bleeding - get it checked out by a specialist - at least then it will reduce the 'what if' questions for you which can often be more scary than the reality.

I would get a new dr, Dr's are supposed to be professional and one who wigs out in my mind is not being professional.
