Children, women and nurses are taught to wipe after defecation from front to back.  To reverse that one brings E. coli (Erischia coli is a common bacteria, up to 60% of the mass of the stool (brown trout, tird, etc is E. Coli)  E. Coli in our gut is actually helpful to us, synthesizing vitamin K.  But in the vagina, it can easily enter a woman's urethra which is only about an inch long where it causes infection.  It generally does not cause infection in the colon.  (O157 H7 is a variant that does but that is rare)  E. Coli is flagellated meaning "it swims."  It has to trouble swimming the inch to the bladder.

If a man fucks you anally (up the hershey highway) and then does you vaginally you stand a high risk of getting an E. Coli infection of the bladder.  E. Coli is the most common bladder infection and not always an STI/STD (Sexually transmitted infection/disease).  The man should use a condom, or at least wash thoroughly and pee after anal intercouse, before moving to the vagina.

Anal intercourse done with loving trust and sensitivity need not hurt at all, and some women can orgasm during it.  The trick is to have the trust and have the woman be literally "open" to the introduction of the male's penis.  Done gently the response usually is "I don't want to do it all the time, but that was hot."

Trichomoniasis is a STI passed from the urethra of the male to the female and back.  It is not generally related to anal intercourse.  As LadyMikaela pointed out it may be the most common STI, but E. Coli infections are far more common, but not necessarily related to sexual acts.  The problem with Trich is that it can be quite asymptomatic and can lead to PID (Pelvic Inflammatory Disease) with little or no symptoms in either gender.  But in the female the results can be infertility.

To go from the anal/rectum to the mouth you will ingest E. Coli.  If you have been with the man for a long time you are likely immune and carry the same strain.  However you do have some risk if there are parasites present.

I agree with NightWindWhisper; having been a nurse in women's health care for more years than I care to list, I can tell you that urinary tract infections are a common problem for women, and E. Coli is the most common bacteria cultured from the urine. The overwhelming cause is contamination from the anus to the urethra. There is only a couple inches distance between anus and urethra, and it is a short climb from the urethra to the sterile climate of the bladder, where the bacteria happily settle into the lining of the bladder walls, before a worsening infection allows them to migrate up the ureters into the kidneys.

Now picture a penis, contaminated with (pardon the pun) a whole shitload of bacteria, thumping and rubbing against the urethra during vaginal intercourse. Emptying the bowel prior to intercourse still leaves the walls of anus and rectum thickly populated with bacteria.

As for as your own E.coli being safe for your own ingestion, I would disagree with that also. E. coli inhabits the lower intestinal tract, not the upper. While healthy people may be colonized by bacteria which do no harm in the bowel, if these same bacteria are transferred to where they are not normally found (urethra, vagina, mouth) infection can and does occur.

Cattle appear to be a reservoir of significant public health importance; however, other
animals, such as deer, are also known to carry E. coli O157:H7. 1n addition, humans may
also serve as a reservoir.
E. Modes of Transmission
Transmission of E. coli shiga toxin-producing infection occurs fecal-orally through food,
drinking water or recreational water contaminated with human or animal feces containing the
bacterium. Transmission may also occur directly from person to person; this can include
certain types of sexual contact (e.g., oral-anal contact). The infectious dose is very low. E. coli
shiga toxin-producing infection has been associated with the consumption of contaminated
foods, such as inadequately cooked ground beef, unpasteurized apple juice and cider,
unpasteurized milk and other dairy products, raw vegetables, and salami. Ham, turkey, cheese
and potatoes have also been implicated in outbreaks.

I realize people will make their own decisions, but I strongly recommend that choices which carry health implications be made after careful research. To engage in an activity because it looks like fun on a porno movie, or because others report they routinely do so and have no problems with it, is not a good idea.

Ok, I'll agree that there is a risk that one "could" get hemorrhagic diarrhea, colitis and hemolytic-uremia syndrome (HUS) .  (O157-H7) is a rare variant of E. coli.  But considering that this strain occurs in ~1.4% of cattle and about the same rate in white-tailed deer, the risk is there, but fairly low.  It's gotta get from the cattle or deer's butt into the human food chain, and sometimes it dose. Consider the discussion in the recent movie Clerks III, the "Ass to mouth" discussion.  Then look at profiles, not here, where kinks are not specifically listed, but try alt, where they are, and rimming (licking the anal area) is quite common.  There is also a risk of getting Liver Flukes from the ova if a person has traveled in the Near East.  There is probably a risk that one might ingest an alien that has taken resident in the person's butt.  Ignoring shiga toxin, or shiga-like toxins and their effects, O157-H7 E Coli is a "reportable disease," and is quite rare as diseases go.  It is often deadly to children and the aged, or immune compromised, and true, a healthy adult can recover and become a "resevoir."  The problem with O157-H7 is that as a flagellated bacteria it can literally "invade" and move into such items as lettuce, and therefore cannot be washed off effectively.  It's problem is that often it has contaminated more than a bit of lettuce, and therefore results in a small epidemic.

So as nightseer points out, it's best not to do.  Keep the shit where it belongs -- however lots of people do ass to mouth, and they don't die like poisoned flies.  In the same vein (pardon the pun) it is best not to perform oral sex on a woman who is menstruating as there is a higher risk of STI's at this time, however probably millions have, and they aren't all dead yet.  It's nice to keep this stuff in perspective.  If you want no risk, sew your cunt up and wear pvc panties (oh, that's right pvc has problems - avoid pvc) are there polyurethane or perhaps ballistic nylon panties available?  Stainless steel---hmmmmm now that sounds interesting.

If you really want to reduce risks, don't exchange fluids until you are a committed couple and have been for at least three months.  Then travel together to a Planned parenthood, get tested for the common STI's and when the results are in, have at it!  Whooooooeeeeeeeeeee....  Exchange those juicky body fluids, hey are we a succulent lot or what?  If you are really avid about rimming, then each licker get tested for parasite and ovum, though it's not terribly definitive because it's hard to detect the ova (eggs).
