I have been involved in planning this same sort of scene, be prepared for lots of blood and have a way to ensure that as the nails are being pulled out of the wood, that they are sterile before being drawn back through the testes, you don't want bacteria to enter either on the way in or out. Ensure that everything is sterilized and kept sterile through the scene.
