For me it's partially because of knowing the person who applies the pain though it's  more on the type of pain being caused and what is being ued. I'd have problems if a wooden spoon as I associate that with corporeal punishment. Yet being bitten hard enough to leave bruises or being flogged fairly rough I will still feel pain but my mind and body interprets that in combination which is then found pleasing and erotic.
