Again, with over 30 years of pastoral care in this area, and a heck of a lot of time spent in the medical field, I honestly haven't seen a lot of this -- nor has my companion (also in the field). While I've seen a FEW players who are what I would consider "extreme", most of the most extreme ones actually eschew going to the ER, because of the attendant risks of jail. I've never seen an admission by individuals who are habitual extreme S/M practitioners, because they KNOW when to stop, and know what they can handle and not handle.
I take that back... I -did- see a couple of cases of what -may- have been erotic autoasphyxiation when I was a paramedic in NY. The surroundings supported that kind of a conclusion, but the individuals were DoA, and it wasn't like we could -ask- them. Erotic asphyxiation, BTW, is one of the most risky activities one can participate in. Autoasphyxiation is dumb. The other big risk is burns (wax play -- which isn't considered 'extreme'... but using the wrong candles/wax...). Most of us who -do- participate in more extreme activities (like my cutting and branding activities) are, for the most part, pretty cautious people -- and a really large proportion of us actually have bothered to do some study in anatomy and physiology so we know how to do what we do with the results we want and minimal damage.
I simply don't understand where all this rant is coming from about the HUGE amounts of resources wasted by S/M practitioners and -extreme- S/M practitioners. Perhaps you have some numbers that would bear this out in your area? How do S/M practitioners' usage of medical resources compare to, say, kids who stick stuff in their ears or noses... or skateboarders... or rock climbers and hikers? How about how they compare to the emergency resources used by families who have their own swimming pools? People who text and drive? People who talk on their cell phones and drive (and those whom they injure by doing so)? People who drink and drive? People who got new shop equipment (particularly saws or drills) for Christmas?
