My thought?  No raised lettering for me, I want inscribed letters, cut a full inch deep into the stone, reading as follows;

[Full Name]
Born [Full Date], [Location]
Died [Full Date], [Location]
son of
[Full Name] ([Years])
and
[Full Name] ([Years])

Boring?  Sure.  But three hundred years down the road, some geneologist will weep with gratitude.

If I were to get a headstone, I'd have the answering machine message set to play when a motion sensor is tripped, with a solar battery to keep it available for play for years and years.

OR

With the same audio setup, I'd have the words, "Come closer.... come closer..." in soft, low tones and then  a scream.

Big blank column. on one side it has the genealogy details. on the others, a recessed shelf to hold a piece of chalk and the words:
YOU FEARS ERASED WITH THE NEXT RAIN.
I figure being dead is no reason to stop being helpful.

In a perfect world my brain would stay frozen in a jar until they could bring me back. Death could be such a setback if I let it be.
